ABOUT EVOLVE

Evolve Health Care is family owned and operated by Mike and Angela Tefft. They started Evolve as a solution to difficulty accessing mental health and straightforward medical problems. Over the years, they have seen clients have difficulty accessing medical services for minor acute problems as primary care providers are unable to schedule them in a timely manner for these types of issues.

Physical Health E-visit (via Text) Services

  • DIAGNOSIS AND TREATMENT FOR ACUTE AND MINOR ILLNESS

  • LABORATORY TESTING

INSURANCE/PAYMENTS

  • We require payment at the time services are rendered.

  • We are not currently in-network with insurance companies. Many insurance companies have out-of-network benefits that can pay 0-100% of the cost of care. We can provide a detailed receipt for clients to submit to their insurer for out-of-network benefits.

  • We accept credit cards and HSA cards for payment.
